If you’re managing a serious health condition, you don’t
have to face it alone. Horizon’s Care Management Plus
program is here to help you better understand your
diagnosis and follow your doctor’s treatment plan.
How it Works
A designated Care Manager, who is a registered nurse,
along with a team of licensed health care professionals,
are available to help you understand complex health
conditions, such as:
• Asthma
• Back pain
• Cancer
• Diabetes
• Heart conditions
• High blood pressure
• Musculoskeletal
disorders
If you have an acute or chronic health condition,
enrolling in the Horizon Care Management Plus
program can help you achieve your best health.
Your Care Manager can:
• Regularly monitor your medical situation and work
with your doctors and caregivers to help manage
your medical needs.
• Talk to you about your health and possible
ways to improve it.
• Helpyoundadditionalhealthcareprofessionals,
including a registered dietitian and behavioral
health specialist, to support your health needs.
• Help you understand your health plan, so you can
getthemostfromyourbenets.
How to Enroll
As an eligible member, you have access to our Horizon Care Management Plus program at no
cost to you, and your participation is voluntary. To speak with someone from the Horizon Care
Management Plus team, call 1‑888‑621‑5894, select option 2, then option 3, weekdays, from
8 a.m. to 5 p.m., Eastern Time.

Care Managers cannot diagnose or treat medical conditions. Members should direct questions and concerns about their health to their physicians. If there is a medical emergency, call 911.
